Things To Do Before Your First Visit

Please bring your driver's license or ID, insurance informationand the referral slip given to you by your dentist.

You can print and complete our forms before coming to the office or fill them out online to save time and paper. If you would prefer to fill out paper copies at our office, please arrive 15 minutes before your scheduled appointment time.

At our first visit, we will review your medical history. Please consider bringing a written list of your medications to make the process go more smoothly. We typically request your x-rays and information about recent dental care from your general dentist, however, we usually need an x-ray or 3D x-ray at our office.

If you or your dentist is unaware of which tooth is bothering you, please avoid taking pain medications such as Advil (ibuprofen) or Tylenol (acetaminophen) before our consultation. Doing so can interfere with our ability to obtain a diagnosis. Feel free to call us if you have any questions!

We do a thorough consultation and for most patients treatment can occur on the same day.
